I worked on a research paper relating to the 'Human Factor' though ofcourse it was related to computer security. Anyway alot of those 'Human Factors' also apply at many other places especially safety critical things like airplanes

I remember there was an incident where someone put this tape on an important sensor which lead to false readings on pilot's dashboard and eventually a deadly crash

I think this is where many of the airlines cut costs where they ignore minor repairs till the scheduled maintainance comes by
