Dartford Crossing payment system still "in beta" and janky af. If you're a very occasional user you can end up in a limbo where the crossing doesn't appear anywhere... but you still get a PCN if you don't pay!

Previously had that dropped I think because they just didn't have time to work out what had gone wrong. Not sure they've yet fixed the bug, or more likely the disjointed processes.
