Insulin was originally (and deliberately) not patented because the creator wanted it to benefit all mankind for free. Today, big pharmaceutical corporations have variants patented and keep the general populace from getting life saving insulin that was meant to be free.

Cocaine is cheap because drugs are easily (and illegally) imported as well as made locally. It's more difficult for the average american to have a decent living income than it is to get hard drugs.
